I would like to borrow the above listed items from Global Outreach for my ministry.  I understand that I am borrowing items which are costly, fragile, and difficult to replace, and which have been purchased with donations from Casas members to promote Global Outreach within the church. I borrow these items at my own risk and agree to return them in the condition in which I received them.  If needed, I also agree to replace or repair any damage to the items I am borrowing. I understand that loaned equipment cannot be taken off campus. I understand that I am responsible for pick-up, check-out, check-in, and return of borrowed items to their storage location.
